White smoke, rough idle, SOMETIMES...

=2000 E-350 runs great some days and like crap others.
-Always heavy white smoke on cold start-up.
-Smells like fuel.
-Usually after it gets warm, it goes away. But sometimes, it will run really rough like its missing a couple cylinders.
-Whenever it runs rough it smokes pretty bad.
-When its running smooth, no smoke.
-Have read over 100 pages of threads. Any help appreciated. thanks

For the rough running I might look at the UVCH connections , also the outers for burned or loose connections.....

Points more to the injectors. Those are typical worn out injector signs if the harness is ok.
