Dealer Sell Back Packet Question

Where do I get one? :shrug: I think after 6 visits to the dealer for the same problems and things only getting worse its time to start the process. My dealer said they dont know what a dsb packet is, and that they would like to keep trying to fix the truck. Currently 05 with 24,750 miles. Been to the shop since it had 6k on it, and would probably have lived there if I didnt have to use it for work. At least it moves. Slow and smokey and burns alot of fuel but it does move. Dealer has tried changing sensors, cleaned the turbo, reprogrammed I lost count, and the engine still runs poorly and struggles to pull my trailer. Its only 9k pounds. Hand calced 12.3 empty highway miles under 65 with a 3.73 rear end, and falling, last tank was 13.6 and I thought that was bad. When I pull my trailer it drops to 6-8 if I dont pull any long hills. I think Ill stop there, I could go on but I have given up on getting it fixed. I test drove an 07, very nice wish I had done it sooner. I didnt realize what I was missing.

Thanks for the info just what I was looking for. Is there a actual form for the dispute settlement or do I just write my own letter and send it in with my repair history.
