Slight miss

The truck seems to have a slight miss. It is very noticable at idle, not so bad when cruising, but very noticible under load ( like when going up hill or while towing).

I have been running 2 stroke oil in the fuel, and use power service every other fill up or so.

The air filter is pretty clean, and it has a new fuel filter on it.

Has a lil smoke when its first started in the morning, and smokes black underload.

Any ideas?
