I own a 91 d-250. It has ol smokey fuel pin just a farm truck. Sometimes I go out and it'll fire right up and other times its wont do anything but spin over. Dont matter how much you crank it will not make any diff. I spray just a SMALL amount of starting fluid in the filter and cranks and runs perfect.??? Any suggestions?

Has to be fuel pressure but I would get a cheap old gauge and throw in between the filter and the IP. Check your cranking pressure and idle pressure and see if you have lost pressure the days it won't start. I think it's probably the lift pump. The VE will pull enough fuel to run but at crank it may just need the help to start.
